Customer lost 2FA device and recovery codes.

Lost-device 2FA recovery requires a manual identity check, for your security, I can't disable it on email confirmation alone.
To proceed, please send:
1. A photo of a government ID matching the account name
2. The original signup email + approximate date
3. Last 4 of the payment method on file (or last invoice number)
Once verified ({{verification_window}} business hours), I'll reset 2FA. After login, set up a new device and store the recovery codes somewhere safe.
